All required intake forms for Binocular Vision Dysfunction (BVD) evaluations are e-mailed securely after scheduling.

Forms are completed online prior to your visit and must be submitted by the deadline provided to hold your appointment.

Appointments are not fully confirmed until required forms are completed.
